Medical Records

By law, every person is entitled to copies of their medical records. This includes lab and test results and doctor's notes.

Release of Medical Records

We will be glad to provide copies of your medical records; however, depending on the amount of pages requested, you may be charged a fee. Most times, you only need one or two pages of your records, and not your complete medical history with our office. All requests for medical records must be submitted in writing. Please give us 3 to 5 days to complete your request.

Medical Records from a Referring Physician

If you are new to our practice, we typically request a copy of your medical records, including recent lab work and imaging tests from your previous primary care provider(s). If they do not provide this information to us in a timely manner, we may request that you obtain these documents for us and bring them with you to your next scheduled appointment.
